MANAGING END STAGE CARDIAC DISEASE AT HOME

A Plus Hospice Care specializes in cardiac care at home.
Our program has been designed to meet the special needs of advanced cardiac patients, we offer a home based option for your care. When a you choose to avoid further re-hospitalizations and your physician has indicated that further surgical procedures would not be effective, A Plus Hospice Care provides a home based option for the medical, physical and emotional support needed to manage the symptoms and realities of heart disease.

The goals of the A Plus Hospice Care cardiac program.
A Plus Hospice Care is committed to you and will participate in your care so that you may stay at home, wherever your home may be, during this stage of your life. We will provide specially trained nursing staff who will collaborate with you and your physician to manage your heart disease with comfort and dignity. Our goal is to manage your symptoms so that you may stay where you want to be.

We work to

  • Increase your ability to cope with your illness
  • Increase family caregiver confidence and offer them emotional support as well
  • Monitor and manage symptoms to bring comfort and reduce any crisis or distress that may arise
  • Improve your quality of life and help you identify and accomplish important goals

Indications that hospice may be the preferred care for you

  • You have expressed interest in staying home and avoiding future hospitalizations.
  • There are ongoing symptoms of breathlessness with exertion or when at rest, swelling in legs, fainting spells.
  • Two or more hospitalizations in past 6 months related to heart condition.
  • The medications to control heart disease are no longer very effective.
  • Ongoing symptoms of heart failure continue even with aggressive use of heart medications.
  • Your physician has given a diagnosis of End Stage Heart Disease. (NY Heart Association Class IV Stage, Ejection fraction <20%, recurring chest pain)
  • The result of various lab tests are abnormal - Hematocrit <35%, Sodium <136 mmol/L, BUN >40, Creatinine >1.8
